Your REHAB Physician will dictate a discharge summary that will be sent to your primary doctors. In addition, our care navigators contact your primary care physician and notify them of your recovery process.
What is the discharge process?
Normally, the day before you leave arrangements will be finalized for; medication for one month, follow-ups with Primary Care Physicians (PCP), Medical Specialists and Outpatient Therapies.
